1°) The layout of the track, its width, nature of the surfacing, access routes and clearance routes reserve ed for spectators and those reserved for the security service;

2°) Placement of the stands with their distance from the track, the nature of their construction, their capacity, places reserved for the public aside from the stands, with their distance from the track, prohibited spaces, the nature of any permanent or special installations to protect the public (crash barriers, wire fencing, mounds of earth, walls, etc...) ;

3°) Placement of all fixed installations: bars, restaurants, etc... ; placement of refuelling stations, security service (firefighters, health service), the spectators’ parking lot and the competitors’ lot.

If the gathering or event is to take place entirely on the road with trials involving private use of a public right-of-way, attach to the proposal a plan of that portion of the road on which the accessory trial will be held, mentioning:

The layout of the reserved road, its width, the nature of its surfacing, places reserved for the public and their distance from the road, prohibited spaces, existing natural protections, special protective installations that will be installed. If the plan described above has already be submitted to the insurers, attach to the proposal only a note specifying the modifications made in the security area since the last gathering or event of the same kind was held.

IN THE EVENT OF OMISSION, INACCURACY OR FALSE STATEMENTS, THE ANSWERS TO THIS PROPOSAL/QUESTIONNAIRE ARE SUBJECT TO THE SANCTIONS ESTABLISHED BY ARTICLES L 113 8 AND L 113 9 OF THE INSURANCE CODE.

The undersigned may ask the Company to notify it of and correct any information concerning it appearing in any file used by the Company, its agents, reinsurers and professional bodies
